Program Expectations and Campus Regulations

Tiger Drummers World Percussion Seminar

 Rules and regulations provide the foundation for a safe and ordered campus life at The Catholic University of America (CUA). The mere observance of these rules and regulations, without the cooperation and personal appropriation of the values that they protect falls short of CUA’s expectations for its campus community. Please discuss this form between parent/guardian and child then complete and return this form to Luis Garay, Program Director of Lion Drummers World Percussion Seminars, held at the School of Music at CUA, no later than the first day of your program.

I, ________________________________, allow my daughter/son/ward _________________________to attend Tiger Drummer World Percussion Seminar on July 26 – July 30, 2010 Monday – Friday from 12:15 pm – 3:15 pm.  I understand that unauthorized departures from CUA campus (including lunch), as well as violations of published campus rules, and may result in termination of her/his attendance in the program. I understand that students who are asked to leave the program will forfeit their entire payment and must have parent/guardian make arrangements for them and that they must leave the Program within 24 hours. I understand that The Catholic University of America assumes no liability for injury, damages or theft arising from the result of participation unless due to willful fault or gross negligence on the part of the University.

I hereby give permission for my daughter/son to receive medical treatment, if necessary, while attending Lion Drummers World Percussion Seminar at The Catholic University of America.
